	Please answer all questions (bottom red link) to the best of your ability. If we feel you are a match for our client, Kai, we will contact you at our earliest convenience. Please allow at least two weeks, or sooner if we can!
	If you wish to apply to become a 4M member, and are NOT interested in meeting Kai, you will need to fill out our general 4M Club Pre-Application form. Otherwise, continue reading and fill out the special form we set up for our client.
	NOTE: Send us at least two pictures—a body and head shot taken within the last six months. This is very important. We don’t want to see what you looked like two years ago! We would also like you to tell us when your photos were taken (month and year), or anything you feel we need to know, i.e. “I recently got my hair cut and expect to have new photos taken within the next couple weeks.” There is a “comments” section on the bottom of the application form for your use, if needed.
	If only sending one photo, make sure it’s at least 3/4 body shot. We don’t accept cropped or group shots, unless it’s all you have for now. You can always send acceptable pictures later, if we request them. Otherwise, we’d like to see two well composed images, of you alone. If you must send a temporary group shot, please let us know which person is you!
